Athlete Requirements
Pattonville High School uses an online program, Healthy Roster, to house all forms needed to compete in MSHSAA sponsored sports and activities such as Marching Band, Drill Team, Cheerleading, Choir, and orchestra. If your student will be participating in any of these activities, you will need to register them using Healthy Roster.
Healthy Roster is an electronic medical record service that is HIPAA compliant, allowing us to securely track our athletes' personal information. All documents will be housed online and accessible to the parents/guardians and coaches. This allows coaches to access emergency information on their smartphones at away games and practices. Healthy Roster will also allow parents/guardians to see any treatments for injuries their student has gone through and provide direct contact with the athletic trainer for updates.
Incoming Freshmen:
- You will receive an invitation email from Healthy Roster asking you to register in March of the previous school year
- Follow the link in the email from Healthy Roster, and you will be able to set up your student’s profile.
Current 9th- 11th Graders:
- If you need a Healthy Roster account or are having trouble logging in, please contact our athletic trainer, Alex Hubbs, at ahubbs@psdr3.org.
Required Forms:
- You will need a physical that is signed and dated by an eligible healthcare provider.
- A reminder for future and current students: physicals will be valid for 2 years ONLY if they are completed on the MSHSAA form and the healthcare provider checks the “2 YEAR” box. Please review your previous physical and be sure it does not A) state it is valid for only one year or B) will expire during your student’s season. Physicals that expire mid-season will cause your student to be ineligible until a new valid physical can be turned in.
- You can print the physical form directly from either the Healthy Roster or MSHSAA website. You can also download the form below. Once you have the form, take it to your doctor to complete. After your doctor has filled out the physical, signed it, and dated it, you can take a picture of the completed form and upload it to Healthy Roster. The physical form is the only form that needs to be uploaded, all other forms are online fillable documents that do not require printing.
- Please check your child’s account frequently, as Healthy Roster will continuously monitor the status of your child’s paperwork and notify you via email when forms are expiring or have already expired.
- For ease of access, please consider using the website version, healthyroster.com on a laptop or desktop, instead of the mobile app.
